- Fingerprint Background Check: Individuals must submit a Fingerprint Based Criminal Background Check Form, a complete set of inked and rolled fingerprints on an Applicant Fingerprint Card, a Payment Authorization Form, and the required payment. Submission can be made by mail or in person. The fee is $20, payable by cash, check, or credit card. Card payments attract an additional $1.00 plus 2.5% of the transaction fee.
